This website provides information about chiropractic care offered at our Tampa, Florida office and lets you request an appointment online. Submitting the new-patient form is a request to be contacted; it is not a guaranteed appointment until our team confirms a date and time with you.
2. SMS Messaging Terms
By opting in through our form, you agree to receive the categories of text messages you selected at the phone number you provided.
- Age requirement. You must be 18 years of age or older to use this SMS service.
- Who sends the messages. 100 Percent Chiropractic.
- Transactional messages may include appointment confirmations, reminders, scheduling changes, and account notifications.
